Please also click below for Student Life Camps, they are only accepting electronic Participant forms this year.
Once at studentlife site, select green Mission camp square. You can then select View Parent Pack and Packing list to print those items. Then select the last box that says Participant Forms (all other locations). There will be an electronic consent form. On the next page, enter 80566 , click green arrow then yellow next document button. Choose Mount Lebanon from dropdown list, complete form and electronic signature.
